- Message frequency varies and depends on your appointments.

- Carriers are not liable for delayed or undelivered messages.
- Opting out of text messages does not affect your ability to receive care. We will simply reach you another way.

4. How Your Information Is Stored
Information you submit through this website is sent to our practice management and communications platform, which stores your contact record and appointment history on our behalf. Providers of that kind are bound to use the information only to deliver the service to us.
5. Medical Information
This policy covers information collected through this website. It is not our Notice of Privacy Practices. Protected health information created or received in the course of your treatment is handled under separate rules, including HIPAA where applicable, and is described in the Notice of Privacy Practices provided at the office. Please do not send detailed medical history through the website forms; bring it to your appointment instead.
